*Sessions where clinical complexity requires longer than the above routine duration, will be billed in increments of $50 per each additional 15 minutes.
Completion of forms, progress reports, letters, or third parties calls on your behalf may incur a prorated fee of $100.00 per hour, billed in 15 min increments of $25.00.
Insurance
We do not bill insurance plans. You will need to file for reimbursement directly from your insurance carrier.
At the beginning of each month, you will receive a super bill to submit to your insurance or health savings plan for direct reimbursement. Please check with your insurance plan for their reimbursement policy for out-of-network providers.
What should I ask my insurance company?
- Does my plan cover mental health services provided by out-of- network providers?
- What kinds of services are covered/excluded, i.e., psychological testing, individual, couples or family sessions?
- Is there a limit to the number of sessions?
- What is the reimbursement rate?
- Is there a deductible? In some plans, you have to pay a certain amount yourself before your benefits start paying.
- How much is the deductible? What services count toward my deductible and when does the deductible starts again?
- How do I submit for reimbursement?
Payment
Fees are due at the time services are delivered. We do not bill insurance plans. You will need to file for reimbursement directly from your insurance carrier.
Cancellation Policy
Full fee will be billed for"No Shows" and Appointments canceled with less than 24-hours' notice.
